Thomas Hogan Centre - Promoting Health Independence (THC)
The THC was officially opened by the Department of Human Services, Hume Regional Director Dr Tom Keating, in June 2005. The THC was created through joining our Coordinating Care Unit (Ground East) and the Thomas Hogan Rehabilitation Unit. The THC now comprises 26 beds specifically dedicated to the needs of people who require active assessment and therapeutic intervention, but whose acute health needs have reduced.
THC provides inpatient sub-acute services for adults with a temporary or permanent handicap, disability of impairment who require interdisciplinary assessment and treatment. People experiencing the impact of a stroke, neurological damage, arthritis, Parkinson's Disease, orthopaedic impairment, mobility restriction, falls and balance difficulties or cardiac/pulmonary disease may benefit from the services provided.
Services are provided by an interdisciplinary team of allied health, nursing, medical and other staff. The focus of sub-acute services is the provision of a goal-orientated, time limited therapy program, based on an individual treatment plan with all patients. A case manager is identified as the key contact person for the patient and their family on admission to the unit.
All potential patients to the unit require a referral from the following sources before consideration for admission will be made: Visiting Medical Officer, General Practitioners, Health Practitioners or other community agency.
A waiting list for admission to the unit is maintained for each program within the service. People can access the inpatient unit whether they are public, compensible, or privately insured.
Programs
Rehabilitation Program
Eligible patients require a focused, intensive rehabilitation program. These patients have significant functional impairment and require the specific skills of a rehabilitation nurse, occupational therapist, physiotherapist, speech pathologist, social worker and/or other members of the rehabilitation team on a daily basis.
Geriatric Evaluation and Management
Eligible patients require multidimensional, interdisciplinary diagnostic assessment and management, designed to determine the medical, psychosocial and functional capabilities of an elderly person, with the intention of developing a comprehensive plan for their therapy and management.
Inpatient Services
- Sub-acute programs are goal oriented, time limited programs.
- Interdisciplinary assessment and treatment plans are individually tailored for each patient.
- Regular reviews of the patients progress against their goals.
- Planning for discharge is commenced as early as possible.
- Each patient has a case manager to assist the patients care planning and delivery.
- Therapy is provided either individually or in a group depending on each client's individual requirements.
