Sub-Acute Ambulatory Care Service (SACS)
The Sub-Acute Ambulatory Care Service (SACS), formerly known as the Community Rehabilitation Centre (CRC) provides centre and home based rehabilitation and specialist continence clinic assessments, diagnosis and treatment plans to people living at home. Services are provided by a multidisciplinary team of allied health, nursing, medical and other staff. Referral to community rehabilitation may occur following discharge from hospital, or referral from General Practitioners and other community agencies. Self referral is also accepted.
Rehabilitation services are provided for adults with a temporary or permanent handicap, disability or impairment such as stroke, arthritis, Parkinson's Disease, orthopaedic conditions, head/spinal cord injuries, amputation, Multiple Sclerosis, mobility problems, falls and balance difficulties and cardiac/pulmonary disease.
Outpatient
Individually tailored programs are developed for each client following a comprehensive initial assessment completed by the rehabilitation team. Goals are established with the client and their family and treatment programs are developed to achieve these. Key contact workers are assigned to each client. Regular review of clients' progress ensures that appropriate treatments are provided. Therapy is provided either individually or in a group depending on each client's individual requirements. - Community Exercise Group
- Orthopaedic Exercise
- Diabetes Education
- Hydrotherapy
- Parkinson's Disease
- Support groups, Parkinson's Disease, Pulmonary, Neuro
- Strength program
