Post Acute Care (PAC)
Post Acute Care (PAC) is a Victorian Department of Human Services funded program for patients admitted to public hospitals.
The aim of the program is to shorten the length of time that people need to stay in hospital, and to prevent unplanned readmission to hospital. This is done by arranging support and services for those who need extra assistance to return home.
PAC can offer additional short term support services when people go home. The aim is for people to recover and return to a normal lifestyle as quickly as possible, and sometimes support services are needed to help this happen. Help from PAC can vary depending on an individuals health and needs. PAC is a free service.
What services can be arranged?
Home Care Services
Assistance with a range of domestic cleaning tasks including vacuuming, cleaning bathrooms, toilets etc.
Personal Care
Assistance with personal hygiene tasks (showering, bathing, dressing etc). To receive this service PAC will need to arrange a personal care assessment by the District Nurse.
Personal Assistance
Assistance with shopping, attending medical or health related appointments.
Respite Care Services
We can arrange for a qualified worker to relieve a carer for short periods while the carer is out of the home.
Transport
The program can arrange transport services to medical or health related appointments if required.
Nursing Care
The program can provide visits from qualified registered nurses to monitor your care at home if required, such as wound care, assistance with medication and other nursing related care. To receive this service you will need a nursing assessment usually before you go home.
Food Services
In exceptional circumstances the Care Cordinator may explore the option of meal provision. Most dietary needs can be accommodated. Availability can vary depending on where you live however every effort will be made to meet your needs.
Allied Health Services
In some special cases the program can arrange allied health services such as occupational therapy or physiotherapy. This is a limited service.
Are there services that Post Acute Care cannot provide?
Some people already receive support services such as Home Help, District Nursing or Meals on Wheels. PAC does not pay for the services that you already had in place before going to hospital, but the program may for a short time increase services available to you.
How long can support be provided?
Support can be provided for up to 28 days after discharge from hospital.
