Palliative Care
NHW Palliative Care provides palliative care services to the shires of Alpine and the Rural City of Wangaratta. Our team consists of Clinical Nurse Specialists, Family Care Coordinator and an Administration Assistant.
The program provides comprehensive assessment including symptom management, access and referral to appropriate services, holistic care coordination, access to equipment, volunteer support, financial assistance and grief & loss support. Referrals can be accepted from health care providers, self or carers.

The Palliative Care Service is primarily a community based service providing inreach support to inpatient facilities across the region.
Information for Clients
Palliative Care is a support service that assists clients living with a life limiting illness. It is a concept of care which provides cordinated medical, nursing, volunteer and allied services for people, delivered where possible in the environment of the person's choice. It provides physical, psychological, emotional and spiritual support for patients and for patients' families, carers and friends. The provision of palliative care services includes grief and bereavement support for the family and other carers during the life of the patient and continuing after death.
